The NHS app is set to be one of the most important tools for delivering the government’s Ten Year Health Plan – making it a one-stop shop for all the information you need about your health and care, no matter which NHS provider is involved in it.


In order to ensure that as many people as possible can make the most of the app, Digital Care Coordinators are working out of several primary care networks in Hampshire and Isle of Wight, including Kay Warne in Eastleigh.


Kay sits with patients, either in the GP practice or their own homes, to get them online, set up and verified, so that they can manage their own health as effectively as possible.
