Wheelchair survey (AJM healthcare)

We are Healthwatch Portsmouth and Healthwatch Hampshire and part of The Advocacy People. Healthwatch is the only independently commissioned voice for anyone who uses local NHS or social care services. 

We are working closely with the HIVE Portsmouth led Disability Advisory Group and other wheelchair user representatives to promote this survey which looks at the quality of local wheelchair services Portsmouth provided by AJM Healthcare.  

We hope you can spare a few minutes to give your anonymous feedback and experience of your NHS wheelchair or Personal Wheelchair Budget service. By completing this survey, you can help shape the future NHS Hampshire and Isle of Wight wheelchair service so that it is better suited to wheelchair users’ needs. Your voice matters, and this is a powerful way to make sure your experiences are heard and acted upon.
