Southern Health NHS Foundation Trust are welcoming feedback from anyone that is a carer, family member or friend of their service users or patients.

The information you provide within this survey will help Southern Health to shape and improve services, and ensure they provide you with support and information to help you in your caring role. The questions have been designed in line with the Service User Led Standards Audits and alongside the Families, Carers and Friends Group.
